Let's review your health plan needs and walk through all of your options together. Have a list of your physicians, current medications, and any coverage concerns handy, and I'll guide you through your choices so you can make the best-informed decision for you and your family. Appointments are by phone or video (Zoom) — just note your preference when you book, and I'll send the right link to confirm. If no preference is given, I'll call the phone number on your appointment. Medicare requires a Scope of Appointment (SOA) form to be completed before we meet.
